This Midterm evaluation feedback form uses basic HTML form features such as text and check boxes. The check boxes will be double checked to make sure only one has been checked per question at a time (interview 2). The top text box allows the student to Enter in the course that they are evaluating, it will be checked against current courses and if it doesn't match then an error page is put back to the users screen with a listing of current courses( questionaire 2 "other" line ). The text box at the bottom of the page can be emailed directly to the instructor if that instructor has enabled that feature, but in every other case the info will be saved along with all of the other information in a database.
This has all been based off a questionaire and interview with one Cari Grindstaff.
All of this feedback is suposed to help the instructor know what he or she needs to change about a course before its too late, but a second interview ( kevin G. ) leads us to something else as well. Saving these midterm evaluations and linking them to the endterm evaluations ( Cari G.'s wishes) and allow the students to rate the instructors improvment. All this can be put out in public for all future students to see when they are registering for classes. Both Cari G. and Kevin G. said YES they wanted these forms to be annonymous and later posted in public.
